It gathers your family, community-based services, and natural support people already in your life together to create and accomplish a plan that allows you to accomplish your goals. Eligibility Requirements
Head of household is a Mecklenburg County resident at least 18 years old.
At least one minor child (from birth to 17 years old) or an 18-year-old dependent high school student must reside in the household.
Participants must be willing to establish a goal based on employment, education, and/or training plus meet with their assigned case manager weekly.
Prospective participants must be willing to identify and work through their personal barriers to achieve identified goals.
Services Offered
Referrals to appropriate community service supports.
Case management (service coordination).
Coaching or Counseling (individual, family, group, youth, and vocational).
Crisis care and outreach.
Education/special education services, tutoring.
Family support, independent support, or self-help .
Psychiatric consultation, community-based in-patient psychiatric care.
Small therapeutic group care, therapeutic foster care.
Employment support.
Childcare assistance for eligible program participants.
Transportation assistance for eligible program participants.
